Selwyn Recreation Discount Verification Form

1. Discounts

Selwyn District Council may offer discounts on memberships, fitness classes and bookings as advertised on the Selwyn Recreation Portal from time to time. Discounts may be available to those who fall within the following discount groups:

• Community Service Card holders
• Hāpai Access Card holders
• SuperGold Card holders
• Student ID holders
• Community Customers

Community customers are customers that:

• (a) are not an individual, and
• (b) have a principal purpose of enhancing the community's educational, social or health wellbeing, and
• (c) are not-for-profit and do not distribute money or property to its members.
• Community customers may include registered charities, unincorporated societies, sports clubs and associations, schools, faith groups and volunteer service organisations.

2. How to establish eligibility for a discount group

• Sign Up for Selwyn Recreation account if you do not already have one.
• Provide all the information requested below via this form. If you prefer, you can provide this information by attending the customer services desk at your nearest Recreation Centre in person.

3. What you need to know

• The Selwyn Recreation Portal will apply standard fees and charges to any purchase made by you until Selwyn District Council has assessed your Discount Verification Form and determined you are eligible for inclusion in any discount group(s) as described in Section 1 above.
• It may take up to five working days for your Discount Verification Form to be assessed. We will advise you if further information is required or if your Discount Verification application is unsuccessful.
• If Selwyn District Council considers you qualify as a member of any discount group(s) this will be recorded in your Selwyn Recreation Portal account. The Selwyn Recreation Portal will automatically apply pricing relevant to your discount group(s). If you are eligible for more than one discount group, the Selwyn Recreation Portal will display the lowest price available.
• Your eligibility for the Community Services Card, Hāpai Access Card or Student ID discount group will end when your card expires. To ensure you remain within these discounts groups, please provide details of any updated card as soon as you receive it. You can do this by resubmitting this form or bringing your new card to the customer services desk at your nearest Recreation Centre in person.
• You may be required to present your SuperGold Card, Community Services Card, Hāpai Access Card or Student ID when you attend any booking made via the Selwyn Recreation Portal.
• Selwyn District Council may ask Community Customers to provide further information to confirm their eligibility for this discount group from time to time.

• evidence of activities as documented on social media accounts or in newsletters, reports, letters of endorsement from other organisations or community leaders.
